Do you Live in Student or Military Housing?

You'll need to bring a proof of address to your first donation appointment. If you live in a dorm or in military housing and do not have a proof of your address, fill out the form below and we'll take care of the rest. Once you receive your card in the mail, please bring it to your first appointment.
